Right-click and choose 'Save As' for any of the following items. Do not view them directly from the site.
Important: To submit the nomination form by email, you must have a default mail client (i.e. Outlook, Outlook Express, Lotus Notes, etc). If you primarily use Gmail, Hotmail, or Yahoo, simply fill out the form and save it to your computer, then manually attach it to the committee. Alternatively, you can press the print button and mail or fax it in. Full instructions are on the form.
